On August 1, 2019, DCS received a report that Parents’ apartment was unsuitable for children; Parents entered into a Safety Plan and cleaned the residence. On September 4, 2019, DCS received a report that the Children were left unattended and that C.K. sustained a four-centimeter gash on his leg, which required stitches. DCS discovered that the Children were “very dirty and had not eaten in a couple of days.” Tr. Vol. II p. 7. C.K.
